Description & Mechanism of Action
FOXO4-DRI (marketed here as Fox 4) is a D-retro-inverso peptide — meaning its sequence is reversed and built from mirror-image D-amino acids, making it resistant to normal enzymatic degradation while retaining its binding activity. It was designed at Erasmus University Medical Center to disrupt the interaction between FOXO4 and p53 inside senescent cells. In senescent cells, FOXO4 normally sequesters p53 in the nucleus, protecting the cell from apoptosis and allowing senescent cells to persist in tissue — a driver of the "senescence-associated secretory phenotype" implicated in aging. By disrupting this FOXO4-p53 interaction, FOXO4-DRI causes p53 to relocate out of the nucleus and toward the mitochondria, triggering cell-intrinsic apoptosis selectively in senescent cells — a "senolytic" effect. In the original 2017 study, treatment restored fur density, renal function, and physical activity in aged and fast-aging mouse models. As of 2026, FOXO4-DRI remains an early-stage academic research construct with no completed human clinical trials.
